    Six-month minimum, then month-to-month. Cancel anytime after.

    • Unlimited primary care visits
    • Direct provider access by cell, text, and email
    • Most routine labs and wellness visits included
    • Same-day or next-day appointments when needed
    • Preventive planning and care coordination with specialists
    • 15% off Althera products and additional services

    Procedures like wound care, sutures, splints, mole/skin tag/cyst removal, and injections are not included in the membership fee but are available to members at 15% off. Emergency care is not provided — please call 911 or visit your nearest emergency department.
